Family & Community Services offers free 'File of Life' kit

Published:

In an emergency every second counts. What medical personnel don't know about your health history may hurt you.

The File of Life is an easy way to ensure that first responders have a quick record of your medical history/conditions/medications if you are unable to provide it yourself.

The kit includes: a notification sticker for the entrance to your home, a magnetic sleeve for your refrigerator and an informational form for you to complete.

These kits are free, thanks to a partnership between Family and Community Services, Inc. and Home Instead Senior Care.

To request a free File of Life kit, call the Medication Assistance Program (MAP) at 330-297-8770.
